The City’s Department of Community and Children’s Services (DCCS) has prioritised the provision of efficient and effective person centred, integrated health and social care services within its Business Plan and its Joint Health and Wellbeing Strategy. To this effect the City will be procuring the services of experienced providers of early intervention and prevention services.
Needs analysis, service gap identification, and market engagement have identified that for DCCS to deliver an improved, integrated and improved outcomes-based early intervention/preventative offer to adults the department should put in place a coordinated supplier infrastructure which will move the department towards developing a system approach with consistent and interlinked outcomes which will further promote independence, delay or reduce the development of health and social care needs and reduce hospital admissions…
